The short answer: Start the Prudential claim through the policy's claims channel. For its current group-life process, Prudential generally requests a completed claim form and death certificate. A trust beneficiary also submits the trust; an estate submits court-issued appointment papers; and a minor beneficiary normally requires a guardian-of-the-estate appointment. How do you submit a Prudential claim?
Confirm whether the policy is individual, employer group, retirement, or annuity coverage; each can use a different form. Prudential's group-life beneficiary form lists email, secure fax, and mail options and tells claimants to include the claim number on submissions.
For most group claims, one beneficiary can provide the death certificate. A certified copy must be mailed for claims above the threshold stated in the current form.
Is the executor automatically the beneficiary?
No. Prudential pays the beneficiary recorded on the policy. The executor acts only when the estate is the beneficiary or when the policy terms and law direct payment there. Obtain the beneficiary determination before assuming proceeds belong in the probate account.
