Voya group life insurance after death: how to start a claim

The short answer: Start at the Voya Claims Center or call 888-238-4840 for life, accidental death and dismemberment, or waiver-of-premium claims. For employer-provided group life coverage, involve the employer's benefits team: Voya's claim packet includes employer information as well as beneficiary paperwork.

Voya's Death Claim for Group Life Plans form asks the employer to complete the group, reporter, and insured information. Its checklist calls for enrollment records, beneficiary changes, and a death certificate showing the manner and cause of death. The beneficiary receives a separate claimant statement.

How do you upload documents and track the claim?
The Claims Center has separate options to start, continue, upload to, and check a claim. Voya's claims checklist says the beneficiary needs the claim number, their birth month and day, and the last four digits of their Social Security number to track a life claim.
Save the confirmation shown after submission. Ask which employer or beneficiary items remain outstanding and who is responsible for each.
